The Effect of Tobacco Smoke on the Tear Film in Active and Passive Smokers: Review

Abstract

<p>Smoking tobacco regularly or being exposed to tobacco smoke for a long time is a risk factor for the development of various problems on the ocular surface, including alterations in the tear and the ocular surface. The objective of this systematic review is to synthesize the available evidence on the various changes that the tear film may present due to exposure to tobacco smoke. For such purpose, clinical studies published between 2016 and 2021 are included,The search was carried out in three databases: PubMed, Web of Science and Science Direct, using the following keywords: "tear film" and "tobacco", as well as the Boolean operator "AND". We compared the values obtained by the TBUT, Schirmer and OSDI tests between smokers and non-smokers. Out of the nine articles included in this systematic review, four of the six that applied the TBUT report a significant difference in the TBUT between smokers and non-smokers. Also, four studies out of six that apply Schirmer I, report that there are no significant differences; The results suggest that exposure to tobacco smoke results in hyperosmolarity of the tear and deterioration of the lipid layer, and as well as change in the measurement of the Schirmer test, and TBUT as indicators of alterations in the quality of the tear film.</p>
